I have never seen it before, so thought maybe you all would also be interested. It was on the third page of the book, in the introduction!
Large pages made it difficult to get a good scan of the entire page, sorry about that.
The title of the book is
"Tony Bennett in the studio-A LIFE OF ART & MUSIC" 2007 and includes a cd with 6 songs
